Care Coordinators Meet June 18, 2015

On Thursday, over fifty professionals responsible for care coordination at the Genf Gesundheitcare Network (GHN) and Tufts Health Plan came together to discuss strategies for managing patients when the newly formed Tufts Health Freedom Plan launches in January 2016. The GHN will provide much of the medical management services for the new company by helping to facilitate the smooth transition of care in the inpatient and outpatient settings, identify and close care gaps, and provide focused treatment for chronic conditions. The group also discussed methods to engage patients in their care as a way to improve communication and produce better outcomes.

Rachel Rowe, Executive Director of GHN notes, “Our care coordinators fulfill a critical role in achieving GHN’s mission of ‘providing high-quality and affordable health care’. These highly trained and dedicated professionals work first hand with patients to ensure preventive measures are taken to avoid unnecessary emergency department visits and to reduce avoidable readmissions. These strategies can improve care to patients and reduce health care costs to the system. Because of this close collaboration, patients have a network of providers that stand ready to assist in all their health care needs.”

GHN regularly hosts learning collaboratives like this one to help achieve the triple aim of higher quality, better access, at a lower cost.
